Abstract

Gastric emptying is frequently abnormally delayed in patients with longstanding, poorly controlled diabetes, and may be associated with upper gastrointestinal symptoms. Delayed gastric emptying may result in impaired glycemic control, particularly in insulin-treated patients, while conversely, acute variations in blood glucose concentration affect the rate of emptying. The pathogenesis of diabetic gastroparesis is heterogeneous, rather than simply being a manifestation of autonomic neuropathy. This chapter will review the clinical characteristics, diagnostic considerations, and management options for diabetic gastroparesis. The latter are currently suboptimal, and an improved evidence base is needed in order to provide the optimum, personalized treatment to individual patients, based on the underlying pathophysiology.
