Abstract
Introduction: Takotsubo syndrome (TTS) has been frequently reported in high-risk patients admitted to intensive care with a broad range of illnesses, medical procedures, and pharmacological therapies. TTS is associated with transient left ventricular dysfunction that can compromise the circulatory system causing harm and life-threatening complications in critically ill patients. Early recognition of TTS can enable appropriate treatment with improved clinical outcomes. Despite three decades of focused research to better understand TTS, there remains a paucity of evidence to describe a universal clinical pathway to detect TTS in these high-risk patients.
Objectives: This review aimed to identify whether any clinical pathways are being used to screen for TTS in patients admitted to the intensive care settings with other medical conditions and treatments.
Methods: JBI scoping review method with three-step search strategy was utilised to search, identify, and synthesise the existing evidence. Databases that were searched include Ovid Medline, Emcare, Embase, Cochrane, CINAHL, PsycINFO, Scopus, JBI Evidence-based Practice Database, ProQuest Central, ProQuest Dissertations and Theses Global.5
Conclusion: The findings have revealed that numerous diagnostic criteria have been developed since 2003 to identify TTS. However, none of the diagnostic criteria identified were utilised on patients admitted to an intensive care setting. Given this finding, an opportunity to develop a clinical pathway specifically to be used in intensive care could assist with early diagnosis of TTS in critically ill patients and to manage TTS safely and appropriately to improve patient outcomes.
Results: A total of 2079 articles from 2000 to 2021 were identified and reviewed. Four studies met the criteria for the scoping review.
