Abstract

In Australia, over 80% of people older than 65 years of age have at least three chronic medical conditions.1–3 By 2050, 6 million elderly Australians will be living with more than three chronic health conditions.1,2 Ninety per cent of people aged over 65 years have at least one medicine-related problem, of which 80% are resolvable. One in five live with a current adverse drug reaction and consequent poorer quality of life; most are resolvable.
