Abstract
Pharmacists play a crucial role in maintaining the safety of patients who request medicines and services from the pharmacy. Provision of medicines and services is not without risk and, from time to time, errors or incidents May occur due to a myriad of possible reasons: inadequate training, lapses in clinical judgement, unexpected pitfalls in systems and processes, to name a few.
