Abstract
Discharging patients from hospital is a complex process that is fraught with challenges.1 Studies have shown approximately half the patients discharged from hospital experience a medical error, with 19–23% suffering an adverse event, most commonly an adverse drug event² Recent Australian research confirms discharge from hospital as a point of increased risk, with 61% of discharge summaries containing at least one medication error.3 Despite awareness of this problem,and increasingly despairing rhetoric regarding the ‘gap in the care continuum’, there has been very little to inspire or improve the patient journey from a pharmaceutical care perspective for the past 20 years.
