Abstract

Discharge of older individuals from hospital is an important issue for hospital and community services’ policy and practice. Devising joint, quality discharge processes between hospital and community services is necessary if the healthcare system is to cope with the impact of the ageing population. In Australia, there is explicit governmental direction for greater collaboration between hospitals and community services, and increased involvement of community services in the discharge process. However, how best to achieve this in practice is largely unknown. There is a paucity of research that explores community services’ involvement in the discharge of older individuals from hospital to the community, or how community services and hospitals can work collaboratively to facilitate increased community services’ involvement in discharge. The overall purpose of this study was to develop a set of Guiding Principles that could be used by hospital and community policy-makers, managers, front-line staff and older individuals to increase the planned and concerted involvement of community services in the discharge process of older individuals from hospital to the community. A staged research approach was adopted as the most suitable method of achieving this purpose. It comprised a systematic review of the discharge literature; expert consensus to develop a list of community services that assist in the discharge of older individuals from hospital to the community; interviews with community services’ staff, hospital staff and older individuals; thematic synthesis of the interview findings to develop the Guiding Principles; and a Show and Tell event to disseminate, and gain feedback on, the research findings and Guiding Principles.
