The diagnosis of osteoporosis is based on bone mineral density (BMD) measurement. The decision to treat is better assisted by the calculation of absolute fracture risk, which incorporates age and clinical risk factors in addition to BMD. Current algorithms for the calculation of fracture risk do not incorporate bone turnover markers since there are inadequate data for their use in risk calculation. Bone turnover markers which show an early response following commencement of treatment are useful for the monitoring of therapy for osteoporosis for which the use of BMD is increasingly being questioned. The use of bone turnover markers helps to identify non-compliance and nonresponders early. Further studies are needed to confirm the utility of bone turnover markers in improving outcomes.
